Sumario: Las enfermedades exantemáticas han sufrido un importante retroceso, debido en gran medida a las campañas de vacunación obligatorias, y a la mejora de las condiciones socio sanitarias. No obstante, se producen brotes de forma esporádica o epidémica, y es importante no olvidarnos de ellas, y ante un proceso febril acompañado de síntomas cutáneos, debemos tratar de llegar al diagnóstico correcto mediante el estudio de las manifestaciones clínicas y de las pruebas de laboratorio, y no conformarnos con calificarlo como un proceso posiblemente de etiología vírica sin filiación concreta, inculpando a los coxsackievirus, echovirus, etc. Junto con los exantemas víricos clásicos como el sarampión, la rubéola y la varicela, repasaremos otros menos conocidos, como el eritema infeccioso, el exantema súbito, la enfermedad mano-pie-boca, la mononucleosis infecciosa, y el síndrome de Gianotti-Crosti, y trataremos también de establecer el diagnóstico diferencial con exantemas causados por rickettsias como la fiebre botonosa, o por bacterias como la escarlatina, o con patologías cutáneas como la pitiriasis rosada en la que se sospecha un origen infeccioso, no suficientemente demostrado.
