Sumario: Objetivos: Estudiar los tipos y patogenia de las reacciones adversas por la vacunación antitetánica. Fuentes de datos: Textos, monografías, comunicaciones, artículos y rastreo bibliográfico en MEDLINE. Selección de estudios: Relacionados con la incidencia, tipos, mecanismos patogénicos y estudio alergológico de las reacciones adversas por el toxoide tetánico. Conclusiones: La inmunización con toxoide tetánico constituye el tratamiento más eficaz para prevenir la elevada morbimortalidad del tétanos. Por ello, la vacuna antitetánica forma parte del calendario vacunal en la infancia, se aconsejan dosis de recuerdo cada 10 años y es considerada como de administración obligatoria para el personal de Tropa y Marinería de las Fuerzas Armadas. Aunque se tolera generalmente bien, son frecuentes las reacciones posvacunación, sobre todo locales y leves. Los efectos adversos se han relacionado con mecanismos inmunológicos, más frecuentes, y no inmunológicos. Las reacciones adversas de tipo inmunológico pueden deberse tanto al toxoide tetánico, como a otros componentes de la vacuna utilizados como conservantes o coadyuvantes. En las personas con antecedentes de reacciones vacunales previas con el toxoide tetánico, debe realizarse un estudio alergológico para determinar su patogenia y evaluar tanto la necesidad como los riesgos de continuación de la pauta vacunal. De forma general, deben establecerse medidas preventivas estrictas para evitar en lo posible los potenciales efectos secundarios de la vacunación antitetánica y en general de cualquier vacuna.
